The organization runs homes for 12 or fewer elderly residents, advocating for a more relational model of eldercare. Residents have private rooms and bathrooms, but share meals in a communal dining room.

During the COVID-19 pandemic, homes run by the Green House Project will have only 32.5 confirmed cases per thousand residents as compared to 146 cases per thousand residents across all nursing homes.
