Conceived of as a form of "hospital reform," members of the social and economic elite, philanthropists, and leaders from local nonprofit organizations are invited to join hospital "boards" to "improve" care and strengthen fiscal management.

This attempt at reform will ultimately fail, as the boards increasingly reflect political interests and alliances rather than the interests of patients and their communities.
