Originally called "Neighborhood Health Centers," the first two centers are founded in Dorchester, Massachusetts and Mound Bayou, Mississippi using funds from President Lyndon B. Johnson's War on Poverty.

The community health centers are intended to provide healthcare to patients who are uninsured or who qualify for Medicaid. Both clinics are founded by physician-activists who have been strongly influenced by social medicine practices in both Latin America and South Africa.
